Physiotherapist - Transitional Care Centre Role

About the Role
We are seeking a skilled Physiotherapist to join our Transitional Care Centre team in West Bedford, Nova Scotia. As a Physiotherapist, you will play a vital role in supporting patients who require additional time, services, and support to transition back to their previous living situation.
Key Responsibilities
- Assess, diagnose, and treat patients in a tertiary care setting who present with actual or potential impairment, pain, functional limitations, or disabilities.
- Encourage, motivate, and assist patients with hygiene and daily living activities, such as bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient's care plan.
- Plan intervention strategies by promoting active involvement of the patient, family, or caregiver in establishing physiotherapy goals which are based on assessment findings, prognosis, and evidence-based practice.
- Participate in interdisciplinary rounds, team meetings, and family meetings regarding patient care, prognosis, and discharge, liaise with medical staff concerning patients' progress and needs.
Requirements
- A master's degree in Physiotherapy.
- Current registration with the provincial College of Physiotherapists.
- Minimum of two years clinical experience.
- Knowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy and their application in specific cases.
