About the Role

We are seeking a dedicated Licensed Practical Nurse to join our Transitional Care Centre team in West Bedford, Nova Scotia.

As a key member of our team, you will be responsible for providing high-quality nursing care to patients who require additional time, services, and support to transition back to their previous living situation.

  • Perform initial and regular health assessments to maintain patient quality of life.
  • Support the development of individualized care plans that focus on the patient's functional abilities and support network.
  • Assess, plan, implement, and evaluate nursing care of patients.
  • Ensure compliance with Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient.
  • Manage patient medication in accordance with legislative requirements.

Requirements

  • Diploma in Practical Nursing and current registration with the Nova Scotia College of Nursing.
  • Comprehensive knowledge of nursing and health care practices, excellent communication, and interpersonal skills.
  • Availability to work a schedule of flexible hours and all shifts inclusive of days, evenings, nights, weekends, and holidays.
