About the Opportunity

We are seeking a compassionate and dedicated Continuing Care Assistant to join our Transitional Care Centre in West Bedford, Nova Scotia. As a member of our team, you will provide personalized care and support to patients who require additional time and services to transition back to their previous living situation.

Key Responsibilities

  • Assist patients with daily living activities, such as bathing, dressing, and mobilizing, according to their care plan
  • Collaborate with the patient and interdisciplinary team to develop and implement individualized care plans
  • Assist with light housekeeping duties and perform delegated nursing acts according to company policy and scope of practice
  • Communicate and report relevant information regarding patient care or safety to Supervisor

About You

We are looking for a candidate who is registered in Nova Scotia's CCA registry, has a valid Continuing Care Certification or Provincial Personal Care Worker course, and can work a flexible schedule inclusive of days, nights, weekends, and holidays. Previous experience supporting vulnerable populations is an asset.
