Job Title: Home Care Nurse

We are seeking a skilled and compassionate Home Care Nurse to join our team at CareHop Nursing & Home Care. As a Home Care Nurse, you will provide high-quality nursing care to patients in their own homes, promoting independence, dignity, and comfort.

Job Summary:

The Home Care Nurse will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing ongoing care and support. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.

Key Responsibilities:
  • Assess patients' physical and emotional needs, and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Document patient progress and communicate with healthcare teams
  • Provide education and support to patients and families on disease management and self-care
Requirements:
  • Current registration as a Registered Nurse (RN) in the province/territory of work
  • Experience in home care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
