Job Title: Home Care Nurse

CareHop Nursing & Home Care is seeking a skilled Home Care Nurse to join our team. As a Home Care Nurse, you will provide high-quality nursing care to patients in their private residences, ensuring their comfort, safety, and well-being.

Job Summary:

We are looking for a compassionate and experienced Home Care Nurse to provide nursing care to patients in their homes. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide nursing care to patients in their private residences, including administering medications, wound care, and vital sign monitoring.
  • Assess patients' needs and develop individualized care plans to meet their unique requirements.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records.
  • Conduct disease screening and provide health education to patients and their families.
  • Manage home care cases, including coordinating with healthcare providers and family members.
Requirements:
  • Current registration as a Registered Nurse (RN) with the relevant provincial or territorial authority.
  • Minimum 3 years of experience in home care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
