Job Title: Home Care Nurse

CareHop Nursing & Home Care is seeking a skilled and compassionate Home Care Nurse to join our team. As a Home Care Nurse, you will provide high-quality nursing care to patients in their private homes, promoting independence, dignity, and comfort.

Job Summary:

We are looking for a dedicated and experienced Home Care Nurse to provide nursing care to patients in their homes. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Provide nursing care to patients in their private homes, including medication management, wound care, and vital sign monitoring.
  • Assess patients' needs and develop individualized care plans to promote independence and comfort.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and communicate with healthcare teams as needed.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current registration as a Registered Nurse (RN) in the province/territory of work.
  • Minimum 3 years of experience in home care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
