Job Description
We are seeking a skilled Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse in Kingston, ON. As a key member of our care coordination team, you will provide and coordinate home health nursing services to diverse individuals, families, and groups according to the College of Nurses of Ontario Practice Standards and program policies.
This visiting nurse opportunity requires driving to support clients with complex care needs and specialized nursing skills at home. You will assess, monitor, and make care planning/coordinating decisions, evaluating care plan interventions through ongoing reassessment and evaluation results. You will also ensure the quality of care by identifying and acting on factors affecting practice autonomy and delivery of quality care.
Key responsibilities include:

You will work closely with clients, families, and healthcare teams to develop and implement personalized care plans, ensuring continuity and consistency of care. Your expertise will enable you to navigate complex care situations and provide specialized nursing services such as wound care, IV therapy, injections, removal of PICC lines, and chemo disconnections.
Required Skills and Qualifications
To be successful in this role, you will need:
- Registration with the College of Nurses of Ontario (CNO)
- RN diploma/degree or RPN diploma from a recognized university or college
- Valid First Aid or CPR Certification
- Experience with wound care or venipuncture knowledge is an asset
- Acute Care and Medical-Surgical nursing experience is an asset
- Knowledge of community nursing is an asset
