Nurse Practitioner Job Description

At 24/7 QUALITY HOME & HEALTH CARE SERVICES LTD., we are seeking a skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement effective care plans.

Key Responsibilities:
  • Assess patients to identify appropriate nursing interventions and develop individualized care plans.
  • Collaborate with healthcare practitioners to provide comprehensive patient care and coordinate care with other healthcare professionals.
  • Monitor patients' conditions and document symptoms and changes in patients' conditions.
  • Provide education to patients and their families on health-related topics and promote healthy behaviors.
  • Administer medications and treatments as prescribed by a physician and manage home care cases.
Requirements:
  • Eligible for licensure as a Registered Psychiatric Nurse by the province of work.
  • CPR Certificate.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and manage a large workload.
