Nurse Practitioner Job Description

At 24/7 Quality Home & Health Care Services Ltd., we are seeking a skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities:
  • Assist healthcare practitioners during examinations or treatments
  • Monitor patients and advise physicians of any changes in patients' condition
  • Address and document symptoms and changes in patients' conditions
  • Assess patients to identify appropriate nursing interventions
  • Collaborate with members of an interdisciplinary health team to plan, implement, coordinate, and evaluate patient care in consultation with patients and their families
  • Conduct disease screening
  • Deliver health education programs
  • Dispense and administer medications and treatments as prescribed by a physician
  • Manage home care cases
  • Operate or monitor medical apparatus or equipment
  • Provide nursing care
Requirements:
  • CPR Certificate
  • Eligible for licensure as a Registered Psychiatric Nurse by the province of work
