At Halton Healthcare, we are seeking a highly skilled and experienced Patient Care Manager to join our Nephrology team. The successful candidate will be responsible for implementing clinical program priorities, overseeing daily operations, and ensuring the provision of high-quality patient care services.

The ideal candidate will have a strong background in nursing, with a minimum of 3 years of experience as a patient care manager in a similar hospital environment. They will possess excellent human resource management skills, including recruitment and retention of staff, as well as excellent budgeting and financial management skills.

The Patient Care Manager will work collaboratively with the team to achieve program priorities and will be instrumental in leading the program's work in the key priorities of the Ontario Renal Network. They will also support partnerships with the ORN, the Interchange project, community partners, and Central Region.

The successful candidate will have a current registration with the College of Nurses of Ontario, a BScN degree, and a minimum of 3 years of recent experience as a patient care manager in a similar hospital environment. They will also possess excellent leadership skills, preferably in Nephrology, and certification in Nephrology is an asset.

The Patient Care Manager will be responsible for planning, implementing, and evaluating activities related to the provision of high-quality patient care services and programs. They will also be responsible for selecting, hiring, and developing staff, and maintaining fiscal responsibility.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with staff, patients, and families. They will also have a strong commitment to continuing education and a demonstrated ability to support team building.

Requirements:

  • Current registration with the College of Nurses of Ontario
  • BScN degree
  • Minimum of 3 years of recent experience as a patient care manager in a similar hospital environment
  • Excellent human resource management skills
  • Excellent budgeting and financial management skills
  • Proven leadership skills, preferably in Nephrology
  • Certification in Nephrology is an asset
