Primary Care Nurse Practitioner

SE Health is seeking a highly skilled Primary Care Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide comprehensive primary care to patients in a Transitional Care Unit (TCU) setting.
Key Responsibilities- Provide high-quality, patient-centered care to patients in the TCU, including health promotion, disease prevention, and management of acute and chronic conditions.
- Work collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments, diagnose, and treat patients, as well as order and interpret diagnostic tests and prescribe medications as needed.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care coordination and transitions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in primary care.
- Collaborate with the Clinical Transition Manager and Transitions Care Lead to achieve key performance indicators and support the achievement of organizational goals.
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care)
- Master's degree in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Nurse Practitioner Diploma from a recognized school of graduate studies
- Minimum 2 years of relevant experience in either home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health
- Demonstrated clinical leadership experience to manage complex patient and family needs in complex settings with need to liaise with an interdisciplinary team and primary care providers
- Excellent assessment skills and a strong clinical background in primary care
- Ability to be flexible and adaptable with excellent organizational skills
- Excellent skills in case management and working with/coordinating interdisciplinary care teams
