Job Summary

The Nurse Practitioner (NP) is a key member of our interdisciplinary team, responsible for providing comprehensive, person-centered care to patients with rehabilitative, palliative, and complex care needs. As a seasoned NP, you will work collaboratively with physicians, nurses, and other healthcare professionals to deliver high-quality care, promote health and wellness, and support patients and families in achieving their goals.

Key Responsibilities
  • Provide comprehensive primary care to patients, including health promotion, disease prevention, and management of acute and chronic conditions.
  • Work autonomously to diagnose, order and interpret diagnostic tests, prescribe medications, and perform procedures within your legislated scope of practice.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans that reflect shared goals and priorities.
  • Promote wellness and health promotion through education, support, and advocacy.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care and safety.
Requirements
  • Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
  • Master's degree in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Nurse Practitioner Diploma from a recognized school of graduate studies required.
  • Minimum 2 years of relevant experience in either home/community care, acute care, chronic disease management, health promotion, palliative care or mental health.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Excellent clinical leadership and communication skills, with a strong focus on patient-centered care and safety.
