We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Tri-County Mennonite Homes. Key Responsibilities:- Provide autonomous practice as a Registered Nurse with Extended Class (RN(EC)) privileges, offering the full scope of primary health care nurse practitioner practice.
- Collaborate with the Medical Director, other physicians, and community services to provide primary health care services to residents.
- Develop, implement, and evaluate resident care plans in collaboration with the interdisciplinary care team.
- Provide education and training to staff, residents, and families on advanced nursing practice issues, including new equipment and technologies.
- Advocate for evidence-based quality health care practice and support team members, residents, and families in advocating for appropriate resources and services.
- Provide leadership and guidance to staff on clinical practice, policy development, and quality improvement initiatives.
- Develop and implement quality activities to maximize service and minimize patient/resident risk.
- Manage care processes to enable each team member to deliver high-quality care and achieve excellence in professional practice.
- Current registration with the College of Nurses of Ontario as an RN(EC), Nurse Practitioner (unrestricted).
- Working knowledge of College of Nurses of Ontario standards of practice.
- Excellent communication, leadership, and problem-solving skills.
- Effective program development, evaluation, resource allocation, and project management skills.
