Job Title: Case Manager/ Case Worker

Job Summary:

We are seeking a highly motivated and compassionate Case Manager/ Case Worker to join our team at Good Shepherd Centres. As a Case Manager/ Case Worker, you will be responsible for providing housing support services to residential and non-residential clients, including case management, crisis intervention, and supportive counseling.

Key Responsibilities:
  • Develop and maintain a caseload of clients, providing individualized support and guidance to help them achieve their goals.
  • Conduct intakes and assessments to identify client needs and develop personalized plans to address those needs.
  • Design and implement interview schedules to evaluate the effectiveness of program participants and make data-driven decisions to improve services.
  • Maintain confidentiality and adhere to strict confidentiality protocols when handling client information and records.
Requirements:
  • A post-secondary education in a related field, such as social work, counseling, or a related field.
  • Experience working in a residential setting, preferably with a focus on housing support services.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, colleagues, and community partners.
  • A commitment to anti-racist and anti-oppressive practices, with a focus on promoting equity and inclusion in all aspects of work.
  • CPR certification and a valid Ontario driver's license (G2) are assets.
