We are seeking a compassionate primary care clinician to join our team at Infinite Medical P.C, a nationwide network of advanced practice providers and specialty clinicians committed to delivering high-quality, proactive care to residents in skilled nursing and long-term care facilities.

About the Role

This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals. As a primary care provider, you will play a crucial role in providing comprehensive care to patients in long-term care facilities.

Key Responsibilities:

  • Provide comprehensive healthcare services and coordinate patient care
  • Work closely with a team of healthcare professionals
  • Assess, diagnose, and treat residents' physical and mental well-being
  • Conduct thorough patient assessments, including medical history reviews and physical examinations
  • Order and interpret diagnostic tests
  • Develop accurate diagnoses and treatment plans
  • Prescribe medications and administer treatments
  • Provide counseling to residents and their families
  • Collaborate with the interdisciplinary team for seamless coordination of care
  • Manage chronic conditions and monitor progress
  • Make appropriate adjustments to care plans

Requirements:

  • Nurse Practitioner or Physician Assistant License
  • DEA License
  • 6 months experience in long-term care

We offer a competitive salary range of $60,000 per year and a commitment to person-centered care.
