Job Title: Cardiac Program Coordinator Assistant

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Cardiac Program Coordinator Assistant.

About the Role:

As a key member of our inter-professional care team, you will play a vital role in facilitating the placement of patients who require cardiac procedures/intervention.

Key Responsibilities:
  • Conduct thorough pre-admission screenings of patients by interviewing, reviewing patient histories, and identifying potential problems.
  • Screen pre-admission/inpatient diagnostic results for completion and identify abnormalities.
  • Communicate potential problems with patients to hospital departments, physicians, and outside agencies as required.
  • Provide support to the Cardiovascular Coordinator in data entry and tracking of patients awaiting cardiac procedures/interventions.
  • Assist in the timely update, recording, and completion of the database for waiting lists and cases.
  • Participate in weekly medical surgical/cardiology rounds and discuss patients with the cardiology groups.
  • Provide patient/family-centered support and educational information to cardiovascular patients.
  • Maintain established systems to notify cardiac and referring physicians of changes in patients' conditions.
Qualifications:
  • Graduation from a recognized program in Nursing.
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years' recent related clinical experience in the designated clinical area, or an equivalent combination of education, training, and experience.
Skills and Abilities:
  • Clinical proficiency/expertise in the designated clinical nursing area.
  • Ability to complete initial and ongoing patient assessment and provide nursing care.
  • Ability to communicate effectively with patients, families/care givers, and the interprofessional care team.
  • Ability to promote person-centered care and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families/care givers about topics essential to their health care and well-being.
