Detailed Overview:
**Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families**:
This position is responsible to prioritize, coordinate and facilitate the appropriate placement of patients requiring cardiac surgery within Royal Columbian Hospital (RCH) and throughout Fraser Health.
In direct collaboration with Cardiac Surgeons and the RCH Surgical Program, is accountable to lead the development, implementation, evaluation and maintenance of a cardiovascular waitlist management system and database to prioritize and facilitate patient placement. Establishes systems to advise/notify cardiologists and referring physicians of patient health status to facilitate completion of procedures within wait-time guidelines; provides patient/family-centered support and education; acts as key liaison for patients and medical staff; ensures the integrated tracking and reporting of statistics related to quality indicators, outcomes, program effectiveness and quality improvement. Develops policies and procedures appropriate to various areas such as patient preparation, efficient transfer of patient cases, tracking of complications, and development of appropriate avenues for follow-up.
**Responsibilities**:
- Establishes systems and processes to assist referring physicians in triage of patients for pacemakers, ICDs (implantable cardiac defibrillator), CRT's (cardiac resynchronization therapy), cardiac catheterization, and cardiac interventions. Ensures referrals are processed in a timely manner and includes complete urgency status, clinical, demographic and contact information.
- Develops, implements and manages an effective cardiovascular waitlist management system for RCH and Fraser Health. Collaborates with Cardiologists, Internists, other Cardiovascular Coordinators and the British Columbia Cardiac Registry in the development, implementation and maintenance of a current, complete and accurate database for all patients awaiting cardiac surgery. Ensures database includes: tracking of patient waiting times and stability while awaiting various procedures related to cardiac intervention and diagnostic catheterization. Maintains current waiting lists, identifying patients exceeding recommended maximum waiting times. Ensures statistics for waiting lists and cases completed are complete and recorded in the database.
- Facilitates continuity of flow for cardiovascular patients from initial booking to post-discharge rehabilitation and follow-up care. Ensures that all appropriate pre-operative diagnostics are complete; appropriate patient results, consultations, and procedure reports are available to the on-call interventionist; coordinates discharge planning. Coordinates transfer of post-operative patients to other hospitals as necessary.
