Cardiac Services Registered Nurse

We are seeking a highly skilled Registered Nurse to join our Cardiac Services team at Royal Columbian Hospital. As a key member of our interprofessional care team, you will provide direct patient care, assess, plan, provide, and evaluate nursing care in an outpatient clinic setting.
Responsibilities- Perform preadmission screening of surgical patients to ensure they can proceed with scheduled surgery.
- Assess patients referred to designated clinical outpatient clinics, collect information from various sources, and make nursing diagnoses.
- Plan, perform, and evaluate nursing interventions and care, coordinate nursing care for patients, and make referrals to the interprofessional care team as needed.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Assess learning needs of patients and families/caregivers, provide information and demonstrations of care activities, and teach patients, families/caregivers about risk reduction strategies, emergency management, use of prescribed medications, self-management skills, lifestyle adjustments, and health maintenance.
- Collaborate with patients and their families/caregivers in developing the care plan, encourage family participation in the development, implementation, and ongoing modification of the care plan, and establish a therapeutic relationship by demonstrating empathy, trust, and respect.
- Participate in the care and discharge planning process for patients, ensure the care plan is established and implemented effectively, update care plans, and intervene as needed.
- Collaborate with members of the interprofessional care team, including physicians and other healthcare professionals/providers, to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Plan and discuss referral information with patients and their families/caregivers, and initiate referrals to appropriate healthcare/community agencies in collaboration with members of the interprofessional care team.
- Participate on nursing committees, care conferences, and councils as assigned or agreed to with the Manager.
- Participate in research and quality improvement activities by identifying patient care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
- Participate in and attend in-services and other educational programs, identify own learning goals, and maintain and update current clinical competence and knowledge within the area of practice.
- Act as a preceptor or mentor to assigned new nursing staff and students, share information, demonstrate care activities, and participate in orientation.
- Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policy/clinical decision support tools, and equipment.
Graduation from an approved Nursing Program recognized by the Canadian Nurses' Association with current practicing registration as a British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years' recent, related experience in the education, monitoring, and management of participants in cardiac prevention and rehabilitation, including exercise training and experience with ECG Telemetry monitoring. ECG rhythm interpretation course.
Completion of or eligible to obtain American College of Sports Medicine (ACSM) Certification.
Current Basic Cardiac Life Support (BCLS) Certification.
Current Advanced Cardiac Life Support (ACLS) Certification.
Valid BC Driver's License.
