We are seeking a highly skilled and dedicated Primary Care Nurse to join our team at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our primary care services.
Key Responsibilities:- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Conduct assessments and provide education to patients, families, and the general public on health-related topics.
- Collaborate with healthcare professionals to ensure continuity of care and promote person-centered care.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
- Comprehensive benefit packages including extended health/dental and a municipal pension plan for part-time and full-time employees.
- Four weeks vacation with one year of continuous service.
