We are seeking a dedicated and compassionate Registered Nurse to join our team in Primary Care. As a Primary Care Nurse, you will be responsible for delivering high-quality patient care and working collaboratively with our interprofessional team to provide comprehensive services to individuals, groups, and communities.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement formal education programs for patients, families, and the general public.
- Collaborate with the interprofessional team to provide person-centered care and promote health and well-being.
- Participate in shift rotation and work Monday to Friday, days, from 08:00 to 16:30.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
- Comprehensive benefit packages, including MSP, extended health/dental, and a municipal pension plan.
- Four weeks vacation with one year of continuous service.
- Financial support for moving expenses is available for eligible positions.
