Job Title: Registered Nurse - Community Care

Job Summary:

We are seeking a highly skilled Registered Nurse to join our team in the MGH2Home Program in Toronto (East York). As a Registered Nurse, you will provide high-quality nursing care to patients in the community, working collaboratively with other healthcare professionals to achieve successful outcomes.

Key Responsibilities:

  • Provide patient-focused, safe, and holistic nursing care to patients in the community.
  • Ensure that nursing care provided to patients is based on current and best practices to achieve successful outcomes.
  • Continually assess, anticipate, identify, and respond to new and changing needs of patients.
  • Travel within your region between patients' homes.

Requirements:

  • Minimum two years nursing experience (Community Nursing experience is ideal).
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community.
  • Proficiency in English, both verbal and written required (additional languages an asset).
  • Valid Ontario driver's license and reliable vehicle.
  • Experience with infusion therapy and wound care.
  • Experience with palliative care and mental health considered an asset.
