Community Nurse

Job Summary:
ParaMed is seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our community nursing team. As an RPN, you will provide high-quality patient care in various community settings, working collaboratively with Registered Nurses (RNs) to support patients with complex needs.
Key Responsibilities:
- Participate in scheduled visits to assess and address patient needs within your scope of practice.
- Provide safe and ethical nursing care, utilizing appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement care plans, monitoring patient progress and adjusting plans as required.
- Engage patients through a collaborative approach, building rapport and trust to promote positive outcomes.
- Maintain accurate and up-to-date patient records, documenting encounters and plans.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in community care is an asset.
- Reliability, flexibility, and a willingness to travel to patient residences within a designated area.
- Passion for making a difference in your community.
