About the Role

Spectrum Health Care is seeking a highly skilled Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our MGH2Home Program in Toronto (East York).

In this full-time contract position, you will work day shifts with regular weekend rotations. As an RN/RPN, you will support client transitions from hospital-to-home, providing high-quality nursing care to patients in the community.

Key Responsibilities

  • Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care team.
  • Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes.
  • Continually assess, anticipate, identify and respond to new and changing needs of our patients.
  • Travel within your region between patients' homes.

Qualifications

  • Minimum two years nursing experience (Community Nursing experience is ideal).
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community.
  • Proficiency in English, both verbal and written required (additional languages an asset).
  • Valid Ontario driver's license and reliable vehicle.
  • Experience with infusion therapy and wound care.
  • Experience with palliative care and mental health considered an asset.
  • Excellent patient assessment and teaching skills.
