Job Title: Social Services Professional

Job Summary:

Halton Healthcare is seeking a skilled Social Services Professional to join our interdisciplinary team. As a key member of our Neuro Rehab team, you will play a vital role in assessing, planning, implementing, and evaluating patient needs. Your expertise will enable stroke/neuro clients to reintegrate into their community setting by providing critical thinking and problem-solving skills.

Key Responsibilities:

  • Conduct comprehensive social work assessments using various modalities and in accordance with professional standards and clinical practices and policies.
  • Consider individual needs and readiness of patient/family/caregiver when providing resources and connecting them to community services.
  • Provide education to stroke/neuro patients and their families regarding their transition back into the community.
  • Collaborate with the inter-disciplinary team to address ongoing patient needs, participate in discussions to optimize patient care, and ensure the coordination of community-based support services.
  • Engage patients to assist with adjustments related to socio/emotional health issues and identify patient's network of supports.
  • Formulate a care plan to accomplish patient's goals.
  • Resolve complex issues with patient/family through conflict resolution, mediation, and advocacy with internal and external partners.

Qualifications:

  • BSW or MSW required.
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
  • Minimum 3 years experience in a hospital or community health organization.
  • Experience working in neurology/stroke setting preferred.
  • Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders.
  • Skillful implementation of planned social work interventions, both individually and in group settings, with the patient, family, caregiver, inter-disciplinary team, and community resources.
  • Able to address unique health, social, and service challenges for the patient/family/caregiver in keeping with Canadian Stroke Best Practice Guidelines.
  • Demonstrates a high degree of initiative, self-direction, and ability to multitask in a fast-paced environment.
  • Demonstrated ability to function effectively as a member of the healthcare team.
  • Satisfactory employment and attendance record.
  • Comprehensive knowledge of community resources and research skills to secure new resources.
  • Care passionately about the needs and well-being of patients, families, and caregivers and the needs of our local community.
