Behavioral Health Program Facilitator

As a key member of our Continuing Care team, the Challenging Behavior Resource Coordinator will play a vital role in enhancing the capacity of continuing care providers to provide care to older persons with complex cognitive and mental health needs. The incumbent will facilitate and support the implementation of the Behavioral Health Program, promoting coordination and integration of services.
About YouWe are seeking a highly skilled professional with a minimum of 5 years' experience in a regulated healthcare profession, including a minimum of 2 years' experience working with clients with responsive behavior in the Continuing Care sector, geriatrics, mental health, and/or ABI populations. Excellent communication and leadership skills are essential for success in this role.
Key Responsibilities- Facilitate the implementation of the Behavioral Health Program
- Enhance the capacity of continuing care providers
- Provide leadership and collaboration to promote coordination and integration of services
- Develop and deliver education programs and facilitate knowledge transfer
