At Northern Health Authority, we are seeking a highly skilled Care Coordinator to join our multidisciplinary team in Quesnel. This is an exciting opportunity for a Registered Nurse to make a meaningful difference in the lives of individuals and families affected by mental health conditions.

Salary: $41.42 - $55.91/hour

Job Description:

We are looking for a dedicated and compassionate RN to provide Assertive Community Treatment (ACT) services to clients with complex needs. As a Care Coordinator, you will work closely with the ACT team to assess client needs, develop individualized care plans, and coordinate community-based services. Your excellent communication and interpersonal skills will enable you to build strong relationships with clients, families, and healthcare providers.

Responsibilities:

  • Conduct comprehensive assessments of clients' physical and emotional needs
  • Develop and implement personalized care plans to promote stability, recovery, and community integration
  • Collaborate with the multidisciplinary team to coordinate community-based services, including housing support, employment assistance, and medical care
  • Provide education and counseling to clients and their families on mental health management and wellness strategies

Qualifications:

  • Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
  • Two years of related experience in the mental health field within the last five years, or an equivalent combination of experience
