Job Summary

SEA MAR Community Health Centers is seeking a highly motivated and compassionate HIV Peer Navigator to join our team. Key Responsibilities
  • Provide one-on-one support and education to patients living with HIV, addressing their unique needs and concerns.
  • Develop and maintain relationships with patients, healthcare providers, and community partners to ensure seamless care coordination.
  • Identify and address barriers to care, including social determinants of health, mental health, and substance use issues.
  • Collaborate with the Rapid ART program director to ensure timely initiation of antiretroviral therapy and maintain connection to healthcare.
  • Communicate effectively with patients, families, and healthcare teams through various channels, including phone, email, and in-person meetings.
  • Stay up-to-date on the latest HIV treatment guidelines, research, and best practices to provide informed support to patients.
  • Participate in regular team huddles, meetings, and training sessions to enhance knowledge and skills.
  • Maintain accurate and confidential records of patient interactions, progress, and outcomes.
Requirements
  • Bachelor's degree in a related field (e.g., social work, public health, nursing) or equivalent experience.
  • 1 year of experience in HIV care, peer navigation, or a related field.
  • Strong understanding of HIV treatment, care coordination, and patient-centered care.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Proficiency in electronic health records (EHRs) and Microsoft Office applications.
  • Valid driver's license and reliable transportation.
Preferred Qualifications
  • Bilingual or multilingual skills (English/Spanish or English/Russian).
  • Knowledge of patient care processes, clinical procedures, and medical terminology.
  • Experience working with diverse populations, including those with mental health and substance use issues.
