About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at ChezRanti Community Centre Ltd. As a key member of our community outreach team, you will play a vital role in providing support and services to individuals in need.

Key Responsibilities
  • Case Management: Develop and implement individualized support plans to meet the unique needs of clients, including assessment, goal setting, and progress monitoring.
  • Community Outreach: Establish and maintain relationships with community partners, agencies, and organizations to ensure seamless service delivery and coordination.
  • Client Support: Provide emotional support, guidance, and education to clients and their families, promoting empowerment and independence.
  • Program Development: Collaborate with the team to design and implement new programs and services, ensuring they meet the evolving needs of the community.
  • Documentation and Reporting: Maintain accurate and comprehensive records of client interactions, progress, and outcomes, using data to inform service improvements.
  • Team Collaboration: Work closely with colleagues to ensure a cohesive and effective service delivery, sharing knowledge and best practices to enhance client outcomes.
Requirements
  • Education: Post-secondary education in a related field, such as social work, psychology, or a related field.
  • Experience: Minimum 2 years of experience in a mental health support role, with a focus on community outreach and case management.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Language: Fluency in English, with the ability to communicate effectively in writing and verbally.
