About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Chezranti Community Centre Ltd. As a key member of our community outreach team, you will play a vital role in providing support and services to individuals in need.

Key Responsibilities
  • Case Management: Develop and implement individualized rehabilitation programs to support clients in achieving their goals and improving their overall well-being.
  • Assessment and Intervention: Conduct thorough assessments to identify clients' needs and develop strategies to address them, including providing crisis intervention and suicide prevention services.
  • Program Development: Collaborate with colleagues to design and implement life skills workshops, substance abuse treatment programs, and behavior management programs that meet the unique needs of our clients.
  • Client Support: Provide one-on-one and group counseling sessions, as well as accompany clients to appointments and social outings to promote their independence and social integration.
  • Community Liaison: Establish and maintain relationships with other social services agencies and healthcare providers to ensure a comprehensive and coordinated approach to client care.
  • Program Evaluation: Assist in evaluating the effectiveness of our treatment programs and make recommendations for improvement.
Requirements
  • Education: College or CEGEP diploma in a relevant field, such as social work or psychology.
  • Experience: 2 years of experience in a mental health support worker role, with a focus on community outreach and rehabilitation.
  • Skills: Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations and build strong relationships with clients, colleagues, and community partners.
  • Language: Fluency in English is required, with the ability to communicate effectively with clients and colleagues.
