About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at ChezRanti Community Centre Ltd. As a key member of our community outreach team, you will play a vital role in providing support and services to individuals in need.

Key Responsibilities
  • Case Management: Develop and implement individualized support plans to meet the unique needs of clients, including assessment, goal setting, and progress monitoring.
  • Community Outreach: Establish and maintain relationships with community partners, stakeholders, and clients to promote awareness and access to mental health services.
  • Service Coordination: Collaborate with healthcare providers, social services, and other community agencies to ensure seamless service delivery and coordination.
  • Client Support: Provide emotional support, guidance, and education to clients and their families, promoting empowerment and self-advocacy.
  • Program Development: Contribute to the development and implementation of mental health programs and services, including workshops, groups, and individual sessions.
  • Documentation and Reporting: Maintain accurate and comprehensive records of client interactions, progress, and outcomes, as well as prepare reports and statistics for evaluation and research purposes.
Requirements
  • Education: College or CEGEP diploma in a related field, such as social work, psychology, or counseling.
  • Experience: Minimum 2 years of experience working in a mental health setting, preferably in a community outreach or case management role.
  • Skills: Excellent interpersonal, communication, and problem-solving skills, with the ability to work effectively with diverse populations and stakeholders.
  • Language: Fluency in English is required, with the ability to communicate effectively with clients and community partners.
