Street Outreach Worker - Service Delivery



About the Job:

We are seeking a skilled and compassionate Street Outreach Worker to join our team. As a key member of our outreach team, you will be responsible for delivering services to individuals in need, providing support and guidance to help them overcome challenges and improve their well-being.

About the Role:

Responsibilities:

  • Deliver outreach services to individuals in need, including those experiencing homelessness, crisis, or trauma
  • Assess client needs and develop personalized service plans to address their challenges
  • Provide emotional support and connect clients with local resources, such as food banks, shelters, and healthcare services
  • Collaborate with other teams and organizations to ensure seamless service delivery
  • Maintain accurate records of client interactions, service delivery, and outcomes


Requirements:

  • Post-secondary education in social work, counseling, or a related field
  • Minimum 2 years of experience in outreach work, crisis intervention, or a related field
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse clients
  • Ability to work independently and as part of a team
  • Valid driver's license and access to a reliable vehicle
