Job Summary

We are seeking a highly skilled and compassionate Foster Care Support Specialist to join our team at WJS Canada. As a key member of our child and family services team, you will play a vital role in supporting foster families and youth in care to achieve successful transitions and outcomes.

Key Responsibilities
  • Conduct regular monitoring visits to foster homes and communities to ensure the well-being and safety of children and youth in care.
  • Collaborate with case management teams to develop and implement comprehensive transition plans that meet the unique needs of each individual.
  • Provide training and support to foster families on competency-based training and professional development.
  • Develop and implement goal-based service plans in partnership with youth and families, ensuring accurate client file management and timely documentation.
  • Ensure compliance with accreditation standards and legislated requirements, including CARF/COA accreditation.
  • Connect children and families with community resources and support networks, and coordinate ongoing service delivery.
  • Provide crisis intervention services and emergency response planning as needed.
  • Utilize standardized assessment and monitoring instruments to ensure continuity of procedures and consistency in follow-up.
  • Participate in multidisciplinary case management meetings, quality improvement, and risk management committees.
  • Perform office-related duties, including data entry, filing, and other administrative tasks.
Requirements
  • 2+ years of experience in case management, working with foster families and youth in care.
  • College diploma in Social Work or Human Services field.
  • Familiarity with accreditation standards for monitoring Foster Homes and Child in Care programs.
  • Preference for completion of the SAFE home study course and working experience, or completion of the 2-day training within the probation period.
  • Valid CPR/First Aid certificate and Non-Violent Physical Crisis Intervention Certificate (MANDT) and Conflict Resolution Certificate.
