Role Overview
Humber River Health is seeking a skilled Registered Nurse to join our Alternative Level of Care (ALC) team at the Reactivation Care Centre – Church site. As a Clinical Care Coordinator, you will play a vital role in delivering high-quality patient care and working collaboratively with our interdisciplinary team.
About the Position
This Temporary Part-Time opportunity offers a competitive hourly rate range of $39.07 - $56.00 per hour. You will work a variety of 8 and 12-hour rotating shifts, including days, nights, evenings, and weekends.
Responsibilities
- Provide exceptional patient care as a Registered Nurse, utilizing your critical thinking skills and prioritization abilities to deliver evidence-based care.
- Care planning and coordination of care with an interdisciplinary team.
- Utilize computerized systems for patient care documentation, ensuring accurate and timely recording of patient information.
- Participate in project teams or hospital committees as required, contributing your expertise and insights to drive quality improvement initiatives.
- Mentorship of new hires and pre-grad nursing students, fostering a culture of professional development and growth.
Requirements
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current BCLS or demonstrated competency required.
- 2 years of recent acute medicine and senior's care experience.
- Critical thinking and prioritization skills required.
- Demonstrated excellent physical assessment skills required.
- Excellent organizational skills with the ability to set priorities, solve problems with evidence-based decision-making practices, and resolve conflicts in a timely and efficient manner.
- Proven experience promoting family-centered care with patient/family teaching skills.
- Ability to work with a multidisciplinary team, patients, family members, and visitors.
- Excellent interpersonal and organizational skills with an ability to set priorities and meet deadlines.
- Basic computer skills required.
- Knowledge of community resources.
- Excellent attendance and discipline-free record required.
