Join Our Team as a Community Facilitator

At Traverse Independence, we offer a unique opportunity to make a meaningful difference in the lives of individuals with acquired brain injuries (ABI). As a Community Facilitator, you will play a vital role in our outreach program, building trusted relationships with clients and supporting them to achieve their goals and improve their quality of life.

Key Responsibilities:

  • Provide support to individuals in their homes or community settings across the Region of Waterloo
  • Work collaboratively with a multidisciplinary team, including regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist, and an Intensive Case Coordinator
  • Develop and implement personalized support plans to help clients achieve their goals and improve their independence
  • Utilize strong organizational, interpersonal, and communication skills to build trust and rapport with clients and colleagues
  • Participate in comprehensive training programs, including internal training and Ontario Brain Injury Association training, to enhance your skills and knowledge in ABI support

Requirements:

  • Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline
  • Demonstrated experience in behaviour management skills is an asset
  • Strong utilization and understanding of Microsoft Office products, particularly M365 - Teams/SharePoint
  • Valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence
