Job Title: Community Facilitator

Traverse Independence is seeking a skilled Community Facilitator to join our team. As a Community Facilitator, you will play a vital role in supporting individuals with acquired brain injuries (ABI) to live independently in the community.

Key Responsibilities:

  • Build trusted relationships with clients and provide support to help them achieve their goals and improve their quality of life.
  • Work in a low-barrier program, providing outreach services to individuals in their homes or community settings.
  • Collaborate with a multidisciplinary team, including regulated staff such as ABI specialists, occupational therapists, and behaviour therapists.
  • Provide support and guidance to clients, helping them to develop skills and strategies to manage their daily activities.
  • Utilize Microsoft Office products, including Teams and SharePoint, to communicate and coordinate with clients and team members.

Requirements:

  • Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.
  • Demonstrated experience in behaviour management skills is an asset.
  • Strong organizational skills, interpersonal skills, and communication skills.
  • Ability to work as a member of a multidisciplinary team.
  • Valid driver's license and access to a reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence.
