About the Role

Traverse Independence is seeking a highly skilled and compassionate Community Facilitator to join our team. As a Community Facilitator, you will play a vital role in supporting individuals with acquired brain injuries to live independently in the community.

Key Responsibilities:

  • Build trusted relationships with clients and provide support to help them achieve their goals and improve their quality of life.
  • Work in a low-barrier program, providing outreach support to individuals in their homes or community settings across the Region of Waterloo.
  • Collaborate with a multidisciplinary team, including regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist, and an Intensive Case Coordinator.
  • Provide support and guidance to clients to help them cope with day-to-day activities and improve their independence.
  • Develop and maintain strong organizational skills, interpersonal skills, and communication skills to effectively work with clients and the team.

Requirements:

  • Community College Diploma in Social Services, Developmental Services, Health Care, or a related discipline.
  • Demonstrated experience in behaviour management skills is an asset.
  • Experience working with people with disabilities, such as mental health and/or addictions, is a great asset.
  • Strong utilization and understanding of Microsoft 365 - Teams/SharePoint.
  • Demonstrated organizational skills, interpersonal skills, and communication skills.
  • Ability to work as a member of a multidisciplinary team.
  • Valid driver's license and access to a reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence.
