About the Role

We are seeking a highly skilled Registered Practical Nurse to join our team as a Behavioural Support Specialist at Langstaff Square Community. As a key member of our healthcare team, you will play a critical role in providing exceptional care to our residents.

Key Responsibilities
  • Provide sound clinical knowledge and skills to lead the Behavioural Support Resource Team (BSRT) in delivering high-quality care to residents.
  • Collaborate with geriatric specialists and other healthcare professionals to develop and implement comprehensive care plans.
  • Coach and mentor staff on the use of frameworks such as PIECES and U-FIRST to assess and manage responsive behaviours.
  • Maintain a short-term active clinical caseload of older adults with complex needs.
  • Provide comprehensive bio-psychosocial geriatric assessments, treatment, and follow-up planning.
  • Document all findings and interventions in a clear and concise manner in the resident's clinical record.
  • Advocate for and protect the individual rights of residents.
Requirements
  • Registered Practical Nurse with current registration with the College of Nurses of Ontario.
  • Current CPR/First Aid certification.
  • Training in Gentle Persuasive Approaches (GPA).
  • Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting.
  • Demonstrated knowledge and skill working with older adults with complex needs.
  • Demonstrated ability to train, coach, and mentor others.
Preferred Qualifications
  • U-FIRST, P.I.E.C.E.S., Dementiability training.
  • Basic computer skills, including Microsoft Word and Excel.
  • Experience with quality improvement methods and processes.
