Job Title: Registered Practical Nurse BSO

Are you a dedicated and compassionate Registered Practical Nurse looking to make a meaningful impact in the lives of seniors? We are seeking a skilled and experienced RPN to join our Behavioural Support Team (BSO) at Langstaff Square Community.

About the Role:

As a Registered Practical Nurse BSO, you will play a critical role in providing high-quality care to residents with responsive behaviours. Your expertise will be instrumental in developing and implementing behaviour care plans, collaborating with interdisciplinary teams, and advocating for the rights and dignity of our residents.

Key Responsibilities:
  • Provide comprehensive bio-psychosocial geriatric assessments and develop individualized care plans
  • Collaborate with the Behavioural Support Team to develop and implement behaviour care plans
  • Coach and mentor LTC home staff in the use of frameworks such as PIECES and UFIRST
  • Provide leadership and support to the LTC home's Behavioural Support Team
  • Convene and chair regular Behaviour Rounds to discuss care plans with internal and external resources
  • Document all findings and interventions in a clear and concise manner in the client's clinical record
Requirements:
  • Current registration with the College of Nurses of Ontario
  • Current CPR/First Aid certification
  • Training in Gentle Persuasive Approaches (GPA)
  • Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting
  • Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours
