About the Role

We are seeking a highly skilled Occupational Therapist to join our ABI Outreach Team at Scarborough Centre for Healthy Communities (SCHC). As a key member of our interdisciplinary team, you will work closely with clients, families, and caregivers to provide comprehensive support and care.

Key Responsibilities
  • Conduct home visits and assessments to identify client needs and develop personalized care plans
  • Collaborate with the interdisciplinary team to provide coordinated care and support
  • Develop and implement client education materials to promote health and wellness
  • Build relationships with community partners and professionals to enhance client outcomes
  • Provide consultation and training to internal staff and community professionals
Requirements
  • Current registration with the College of Occupational Therapists of Ontario
  • Proficiency in the use of computers and EHR applications
  • Knowledge of the healthcare system and ABI preferred
  • Excellent communication and partnership-building skills
  • Demonstrated commitment to continuous learning and professional development
