About the Role

We are seeking a highly skilled Occupational Therapist to join our ABI Outreach Team at Scarborough Centre for Healthy Communities (SCHC). As a key member of our interdisciplinary team, you will work closely with clients, families, and caregivers to provide comprehensive support and services.

Key Responsibilities
  • Conduct home visits and assessments to identify client needs and develop personalized care plans.
  • Collaborate with the interdisciplinary team to provide coordinated care and support.
  • Develop and implement client education materials to promote health and wellness.
  • Build relationships with community partners and healthcare professionals to enhance client outcomes.
  • Provide consultation and training to internal staff and community professionals to enhance skills and knowledge.
Requirements
  • Current registration with the College of Occupational Therapists of Ontario.
  • Competencies in oral and written communication, problem-solving, and critical thinking.
  • Proficiency in the use of computers and EHR applications.
  • Knowledge of the healthcare system and ABI preferred.
