Occupational Therapist Case Manager

We are seeking a highly skilled Occupational Therapist to join our ABI Outreach Team as a Case Manager. The successful candidate will work collaboratively with our interdisciplinary team to provide comprehensive care and support to individuals with Acquired Brain Injury and their families.
Key Responsibilities- Conduct home visits and assessments to identify client needs and develop personalized care plans
- Collaborate with the interdisciplinary team to provide coordinated care and support
- Develop and implement client education materials to promote health and wellness
- Build relationships with community partners and healthcare professionals to enhance client outcomes
- Provide consultation and support to internal staff and community professionals
- Current registration with the College of Occupational Therapists of Ontario
- Competencies in oral and written communication, problem-solving, and critical thinking
- Proficiency in the use of computers and EHR applications
- Knowledge of the healthcare system and ABI preferred
This is a full-time position with standard working hours and flexibility to accommodate occasional evening or weekend appointments. Travel throughout Scarborough is required, and access to a vehicle is an asset.
RemunerationThe successful candidate will be offered a competitive salary range of $33.04 - $39.80 per hour, with benefits and pension plan membership.
