About the Role

We are seeking a highly skilled Occupational Therapist to join our ABI Outreach Team as a Case Manager. The successful candidate will work collaboratively with our interdisciplinary team to provide comprehensive care and support to individuals with Acquired Brain Injury and their families.

Key Responsibilities
  • Conduct home visits and assessments to identify client needs and develop personalized care plans.
  • Collaborate with the ABI Outreach Team to provide coordinated care and support services.
  • Develop and maintain relationships with community partners and healthcare professionals to enhance client outcomes.
  • Provide education and support to clients and their families to promote independence and well-being.
Requirements
  • Current registration with the College of Occupational Therapists of Ontario.
  • Experience working with individuals with Acquired Brain Injury and their families.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
