We are seeking a dedicated and compassionate Housing Support Worker to join our Community Homes for Opportunity (CHO) Program. As a key member of our team, you will work collaboratively with clients to provide individualized and flexible support and wellness planning, helping them maintain their housing, engage with their community, and achieve their goals.
Key Responsibilities:- Provide support and guidance to clients in their homes and community settings
- Develop and implement individualized wellness plans to promote client independence and recovery
- Collaborate with clients, family members, and other stakeholders to identify strengths and needs
- Participate in individual and systemic advocacy initiatives to improve resource access and availability
- Work closely with the CHO Housing Administrators and Home Operators to ensure seamless service delivery
- Degree or diploma in health or social services
- Current Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Experience working with adults with serious mental illness in community-based settings
- Knowledge of the Ontario Mental Health Act, mental health reform principles, and relevant legislation
- Ability to apply recovery principles and empowerment-oriented philosophies and practices in work with clients
