About the Role

We are seeking a Housing Support Worker to join our Community Homes for Opportunity (CHO) Program. As a Housing Support Worker, you will work with clients to provide individualized and flexible support and wellness planning. Your focus will be on helping clients maintain their housing, engage and integrate them into the community, and access resources and supports to maximize their independence and potential.

Key Responsibilities

  • Assist clients in identifying strengths and needs, and develop long-term goals, short-term objectives, and concrete action steps to achieve these objectives through wellness plans.
  • Participate in individual and systemic advocacy initiatives to improve resource access and availability.
  • Work collaboratively with the CHO Housing Administrators and Home Operators as part of a multi-disciplinary team.

Requirements

  • A degree or diploma in health or social services.
  • Current Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Experience working with adults with serious mental illness in the community, preferably in congregate living environments, Homes for Special Care, or Housing with Supports.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act, and PHIPA requirements.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.

Working Conditions

Office work, community settings, meetings in clients' homes, and travel with clients.
