Job Title: Housing Support Worker – Community Homes for Opportunity Program

We are seeking a dedicated and compassionate Housing Support Worker to join our Community Homes for Opportunity (CHO) Program. As a key member of our team, you will work collaboratively with clients to provide individualized and flexible support and wellness planning, helping them maintain their housing, engage with their community, and achieve their goals.

About the Role:

The Housing Support Worker will work in a combination of office and community settings, providing support to clients in their homes and facilitating access to resources and services. You will work closely with our CHO Housing Administrators and Home Operators to ensure seamless delivery of services and support.

Responsibilities:
  • Provide individualized and flexible support and wellness planning to clients
  • Help clients maintain their housing and engage with their community
  • Facilitate access to resources and services to support client goals
  • Work collaboratively with the CHO team to ensure seamless delivery of services and support
Requirements:
  • Degree or diploma in health or social services
  • Current Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Experience working with adults with serious mental illness in the community
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, and relevant legislation
  • Ability to apply recovery principles and empowerment-oriented philosophies and practices in work with clients
