Island Health: Long Term Care Case Manager

At Island Health, we are seeking a highly skilled and compassionate Long Term Care Case Manager to join our team. This is an exciting opportunity for a dedicated healthcare professional to make a meaningful difference in the lives of our patients.

Job Summary:

We are looking for an experienced Nurse with a strong background in gerontology to provide high-quality care to our patients. The successful candidate will have excellent clinical skills, strong communication abilities, and a passion for delivering patient-centered care.

Responsibilities:
  • Clinical Competence: The successful candidate will demonstrate clinical competence in areas such as chronic disease management, health promotion, and illness prevention.
  • Inter-professional Collaboration: The case manager will work effectively as part of an inter-professional health care team, collaborating with other staff, programs, and organizations to deliver comprehensive care.
  • Problem-solving and Conflict Resolution: The successful candidate will be able to resolve conflicts, problem-solve, and build consensus to achieve positive outcomes for our patients.
  • Communication and Relationship-building: Excellent communication and relationship-building skills are essential for this role, as the case manager will need to interact with patients, families, caregivers, and other healthcare professionals.
  • Travel and Transportation: Travel is a requirement of this position, and the successful candidate must have reliable transportation to meet the operational requirements of Island Health.
Requirements:
  • Education and Training: A degree in Nursing or a related field is required, along with registration with the BC College of Nurses and Midwives as a practicing RN registrant.
  • Experience: Three years of recent related experience in gerontology, including two years of community health experience, or an equivalent combination of education, training, and experience.
  • Skills and Abilities: Strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.
