Mental Health Case Worker Specialist

We are seeking a skilled RN/RPN Case Worker to join our team at Island Health. This is a dynamic role that requires a comprehensive understanding of mental health, excellent communication skills, and the ability to work effectively in an outreach setting.
Key Responsibilities- Provide high-quality care to individuals with affective disorders, schizophrenia, and other serious psychiatric conditions.
- Work collaboratively with multidisciplinary teams to develop and implement patient-centered plans.
- Conduct thorough assessments, identify needs, and coordinate services to meet client goals.
- Develop and maintain effective relationships with clients, families, and community resources.
- Bachelor's degree in Nursing or equivalent experience.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Minimum 2 years of recent experience in mental health case management or a related field.
- Valid Class 5 vehicle license.
- A competitive salary of $73,000 - $90,000 per year, commensurate with experience.
