About the RoleThis exciting opportunity as a Discharge Planner Social Worker offers the chance to make a meaningful impact on the lives of patients and families navigating complex medical situations.
You will work collaboratively with an interdisciplinary team to facilitate timely transitions from the hospital system, ensuring patients receive the right care at the right time.
Key Responsibilities- Crafting personalized discharge plans that meet the unique needs and goals of each patient and their family system.
- Providing critical thinking and creative problem-solving skills to address complex discharge planning scenarios.
- Collaborating with internal and external partners to identify and address barriers to discharge, ensuring a smooth transition to the community.
- Conducting thorough assessments to determine the need for psychosocial interventions and providing expert guidance throughout the implementation process.
To excel in this role, you will possess:
- A Master's degree in Social Work (MSW) or a Bachelor's degree in a Health Discipline, along with current registration and membership with the relevant college.
- 2-5 years of clinical experience in an acute care hospital setting, with a focus on discharge planning and psychosocial assessments.
- Excellent communication, collaboration, and problem-solving skills, with the ability to work independently and as part of a dynamic team.
- Demonstrated knowledge of community resources, legislation governing public hospitals, and the impact of social determinants on discharge planning.
The salary for this role is approximately $44.67-$57.90 per hour, based on the Allied Health (AH10.6) role level and location in Ontario.
