Job Title: Shift Nurse

Job Type: Full-Time, Part Time, and Casual
Closing the Gap Healthcare (CTG) is a leading provider of home and community care services in Canada. We are seeking a skilled Shift Nurse to join our team and provide high-quality care to our clients.
About the Role:
- Provide wound care management and treatment to clients in their homes or community settings.
- Administer IV antibiotics, cannulation, chemo disconnects, CVAD care, and PORT-A-CATH care access/de-access as required.
- Coordinate with healthcare teams to ensure seamless care delivery and effective communication.
- Travel to clients' homes or community settings to provide care and support.
- Document client progress and maintain accurate records.
- Order supplies and equipment as necessary.
- Build strong relationships with clients and their families to provide personalized care and support.
- Work independently and as part of a team to achieve client goals and outcomes.
- Stay up-to-date with industry developments and best practices to provide evidence-based care.
- Ensure client safety and well-being at all times.
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) certification with the CNO.
- Minimum two years' experience in community or clinical/hospital setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Current Standard First Aid & CPR/AED (Level C) certification.
- Current Mask Fit test.
- Ability to work in a fast-paced environment and adapt to changing priorities.
