Job Summary

Spectrum Health Care is seeking a skilled Nurse Care Specialist to join our team of healthcare professionals. As a key member of our community health team, you will be responsible for providing high-quality nursing care to patients in their homes.

Key Responsibilities

  • Provide patient-focused, safe, and holistic nursing care to patients in the community.
  • Work collaboratively with other members of the healthcare team to ensure seamless care delivery.
  • Continually assess, anticipate, and respond to new and changing needs of patients.
  • Travel within your region to provide care to patients in their homes.

Requirements

  • Minimum two years of nursing experience, preferably in community nursing.
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Ability to work independently in the community.
  • Proficiency in English, both verbal and written, required (additional languages an asset).
  • Valid Ontario driver's license and reliable vehicle.
  • Experience with infusion therapy, mental health, palliative care, and wound care.
  • Excellent patient assessment and teaching skills.
